Telfast Review Article

 

Telfast (generic name fexofenadine hydrochloride), manufactured by Sanofi Aventis and marketed as Telfast and Allegra, is a type of medicine called a non-sedating antihistamine which works by preventing the actions of histamine - a substance produced by body as a part of its defence mechanism. Telfast is available in tablets of 60 mg, 120 mg or 180 mg and is used to treat seasonal allergic rhinitis. It should not be used in children under 12 years of age, in breastfeeding or if the patient is allergic to one of it's ingredients. Telfast reduces the symptoms of hayfever (seasonal allergic rhinitis) and other allergies. Allegra (Telfast), like other antihistamines, works by preventing the action of histamine on your nose and eyes. Telfast is used for relief from the symptoms of allergies such as runny nose, itchy and watery eyes, itchy throat, and sneezing.

Telfast is an antihistaminic drug whose main active ingredient is called Fexofenadine hydrochloride. The main use of Telfast is in the treatment of hayfever and other similar allergies like perennial allergic rhinitis. It can also be used to relieve the symptoms of hives or chronic idiopathic urticaria (also known as nettle rash).

Histamines & Allergic Reactions

Telfast is a medicine that belongs to a group of drugs called antihistamines. This means that Telfast and other antihistaminic drugs work by countering or treating the symptoms that are caused by histamines. Histamines are organic nitrogen compounds that are responsible for local immune responses. If a foreign body is introduced to your organism the histamines are produced as an immune response.

If your body is therefore allergic to certain substances then you will have an allergic reaction when exposed to these substances thanks to the histamines. Antihistamines like Telfast will therefore reduce the allergic symptoms.

Most of these allergies will appear in seasons every year. It is seasonal like this due to the fact that in these seasons of spring and fall there are a countless number of pollen grains in the air. For a certain percentage of those who have allergies, as time goes by the condition seems to get worse every year. This is what is known as the priming effect and an antihistamine like Telfast can treat it and prevent further priming.

Common Conditions Treated With Telfast

Hay Fever
Hay fever is an allergic condition that is also known as allergic rhinitis. The symptoms of hay fever practically resemble those of a common cold. The patient would have a runny nose, congestion and would also be sneezing. 1 out of 5 people are affected by this condition caused by allergens like pollen and pet fur or dander. The condition is called hay fever due to the fact that the allergies occur during the season of making hay.

Perennial Allergic Rhinitis
Perennial allergic rhinitis is also an allergic reaction practically similar to Hay fever. The symptoms are the same and the causes of the allergic reaction are also the same being allergen substances like pollen. The only difference between Hay fever and perennial allergic rhinitis is that hay fever is from seasonal occurrence while perennial allergic rhinitis will manifest itself all year round. It is usually brought about by indoor allergens which are substances like dust, pet fur and mold.

Chronic Idiopathic Urticaria
Chronic idiopathic urticaria is a condition most commonly referred to as hives. It can be described as an itching that keeps on occurring daily without the cause as to why being so obvious. The hives can occur daily for even up to 6 weeks. It comes about as an allergic reaction to some sort of allergen like pollen or poultry products. Treatment with antihistaminic drugs like Telfast can resolve the condition.

Actions

There have been many various studies carried out to determine the effectiveness of antihistamines like Telfast in the treatment of the different types of allergic rhinitis. One of these clinical trials was done as recently as 2009. In the review written after the clinical trials there were a couple of new results discovered from the investigations done. The new results come from the clinical application of H3 and H4 receptors in therapy of diseases like allergic rhinitis. As part of the research the patients were subjected to treatment with the use of placebos of which were then compared to the reference drug (Telfast).

The trials showed that there have been improved safety standards with the drug and its effectiveness has also gone up considerably. This has all been made possible with the possibility of combining the antihistaminic drug with inhaled glucocorticosteroids and antileukotrienes. The trials also looked into the advantages that nasal antihistamines would have over tablets due to their increased concentration levels in inflamed tissue. All in all the review proves that antihistamines like Telfast can still be highly and constantly improved for the progress of treating allergic diseases.

According to yet another clinical trial carried out to see the effectiveness of antihistamines on treating allergies in children, it was found that second generation antihistamines have a better risk to benefit ratio than the first generation antihistamines.
This means that the first generation antihistamines should therefore be limited if not completely avoided when it comes to the treatment of allergies in children.

Telfast is described as both a second-generation as well as third-generation antihistamine. It is recommended that it can only be given to children of 12 years or older at a dose of 60mg twice daily or 180mg once a day. The clinical trial showed that Telfast (Fexofenadine) was found to bring about more satisfactory results in the treatment of childrens allergies than a majority of the other antihistamines in the market. These results factor in the overall efficacy of Telfast and the tolerability without forgetting the fact that Telfast unlike many of the other antihistamines is a non-sedating antihistamine. This means that the children can take it and not have to suffer any decline of their activeness with school or other activities due to drowsiness or sleepyness.

Indications of Telfast

Telfast is mainly recommended for relieving the symptoms of three allergic reactions from the following diseases:

1. Seasonal allergic rhinitis also known as hay fever
2. Perennial allergic rhinitis
3. Chronic idiopathic urticaria

Dosage and Administration of Telfast

Hay Fever/ Seasonal Allergic Rhinitis

Children of ages 2-11 years old - 5mL (this is equivalent to 30mg if you have only tablets).

Children from 12 years old and above/ adults 120mg or 180 g once daily.

Urticaria
For ages 12 and up 180mg of Telfast once a day.
Adverse Effects of Telfast
Telfast is one of those few medicines that does not have dangerous or extremely untolerable side effects. The following are the most common adverse effects of telfast:
Mild headaches
Fatigue
Dizziness or drowsiness
Nausea

Overdosing
There are not too many reports of patients overdosing on Telfast and as a result therefore the effects are not that well established. There were many trials run on mice with 5,000mg/kg bodyweight and there were no deaths recorded. This dosage is 110 times the maximum recommended dose that can be given to an adult human. When the same tests were done on rats as well there were once again no deaths shown despite the dosage being about 400 times that which can be recommended to an adult human.

Pharmacokinetics:One to three hours after the administration of Telfast the plasma concentrations of the drug will be at their maximum. The maximum effectiveness is however put at 6 hours after administration. About one third of the medicine is absorbed into the bloodstream with a very small percentage being metabolized by the liver due to the methyl ester and azacyclonol that are pharmacologically irrelevant.
