Vepesid generic name etoposide (etoposide phosphate) is used as a form of chemotherapy for malignancies such as Ewing's sarcoma, lung cancer, testicular cancer, lymphoma, non-lymphocytic leukemia, and glioblastoma multiforme. It can also be used in combination with other cancer medications. Vepesid is also sometimes used in a conditioning regimen prior to a bone marrow or blood stem cell transplant. Do not use this medication if you use any kind of dietary supplements or if you have kidney disease, bone marrow depression or a history of blood conditions. Vepesid capsules absorption depends on the severity of disease. Total dose of vepesid should not exceed 650mg/m2.
Vepesid (etoposide) is indicated for use in the treatment of:
1. Small cell carcinoma of the lung.
2. Acute monocytic and myelomonocytic leukaemia.
3. Hodgkin's disease.
4. Non-Hodgkin's lymphoma.

Vepesid is an inhibitor of the enzyme topoisomerase is an anti cancer agent. It is chemically derived from podophyllotoxin found in American Mayapple. The basic usage of this drug is for the treatment of lung cancer, testicular cancer, lymphoma. Ewing's sarcoma, glioblastoma multiforme and non lymphocytic leukaemia in the form of chemotherapy. Vepesid is also advantangeous in regimen condition prior to bone marrow or blood cell stem transplant. Vepesid along with certain other drugs is also recommended.
Vepesid is given in the form of capsule or sometimes intravenously. When given through veins (intravenously), the process duration should be of 30 to 60 minutes as it can lower the blood pressure. So it become necessary to keep on checking the blood pressure during infusion. The IV may result into infection or pain at the injection site. It is advisable to drink plenty of water and fluids afte treatment to avoid damage to the bladder and kidneys. The recommended intake of water is 1.5 to 3.5 on the treatment day.
Several studies conducted to analyze the effect of vepesid have revealed that this drug initially causes arrest of metaphase, but with the due course of time the effect is replaced by the interference of cell cycle progression prior to the cell entering mitosis. In an another important study using human lymphoblast it has been shown that cell life progression and maximum of cell killing occurs in the S and G2 phases of the life cycle. The same has been confirmed by several cell lines. According to the researachers, the mechanism behind this can be related to an inhibition of nucleoside transport demonstrated in HeLa cells. Vepesid is typically active in human leukaemia cells and a high response rate is also seen in small cell carcinoma of the lung.
Indications And Contraindications
Vepesid is indicated for the treatment of hodgkin's disease, small cell carcinoma of the lung, acute monocytic, myelomonocytic leukaemia and non hodgkin's lymphoma. It is highly recommended not to take this drug by the patients with severe hepatic dysfunction, have demonstrated hypersensitivity to any of the infredients of the drug and severe bone marrow failure not due to malignant disease.
Intravenously the drug should be given at 50 -60 mg/ m2/day in adults for 5 days followed by a interval of 2-4 weeks. Care should be taken that the dose should not exceed 400mg/m2 per course. As mentioned earlier, the infusion should not be given with rapid push. The solution of vepesid should be administered immediately after reconstitution within 24 hours. The vepesid injection should be use once and the residue should be discarded. The recommended starting dosage of vepesid is approximately 100-200 mg/m2/day for 1 to 5 days.
Store vepesid at room temperature. No proteection needed from normal room fluorescent light.

Vepesid side effects include fast heartbeat, loss of consciousness, shortness of breath, sweating, swelling of face or tongue, tightness in throat, wheezing, unusual tiredness or weakness, black/ tarry stools, blood in urine or stools, cough or hoarseness accompanied by fever or chills, lower back or side pain, painful or difficult urination, pinpoint red spots on skin, sores in mouth or on lips, unusual bleeding or bruising, loss of appetite, nausea, vomiting and temporary loss of hair.
Etoposide mechanism of action results by exerting its anticancer effects by inhibiting the enzyme topoisomerase, which ultimately leads to un-repaired breaks in cellular DNA.
Vepesid injection contains no antimicrobial agent. Use once only and discard any residue. Vepesid capsules absorption is variable. Total dose of Vepesid should not exceed 650mg/m2 per course. It should be administered and consumed under the supervision of a qualified physician experienced in the use of cancer chemotherapeutic agents. If a dose is missed, it is advised to consume it as soon as it is remembered. If it is near the time of the next dose, the missed one should be skip and the usual dosing schedule should be resume.
This medicine can cause birth defects if being used at the time of conception or during pregnancy. Before taking etoposide makes sure your doctor knows if you are pregnant or if you may become pregnant. Breast-feeding is generally not recommended while you are receiving it.
Store Vesipid at room temperature in the container it came in and out of reach of children.
